These Terms of Service ("Terms") govern your access to and use of our website located at https://rejuvsf.com (the "Site"), our online booking systems (including Vagaro), text messaging programs, and aesthetic or wellness services provided at our clinic located at 255 King Street Suite A, San Francisco, CA 94107.

By accessing the Site, scheduling an appointment, purchasing treatment packages or toxin banks, or enrolling in our communication channels, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use our Site or services.

1. Medical Disclaimer & General Informational Nature of Website Content

The content on our Site, including blog posts, procedure descriptions, treatment guides, before-and-after imagery, and educational materials, is provided strictly for informational purposes. It does not constitute medical advice, medical diagnosis, or a formal patient-provider relationship.

Consultation with a qualified, licensed healthcare professional at REJUV Medical is required to evaluate your individual health profile, contraindications, and aesthetic candidacy. Individual results vary based on age, genetics, metabolism, baseline skin condition, and lifestyle factors.

2. Appointments, Scheduling & Vagaro Online Booking

Appointments may be booked through our Vagaro scheduling portal, via phone at (415) 712-1937, or by direct communication.

• Card on File: To secure your appointment, a valid credit or debit card must be placed on file at the time of booking.
• Confirmation: You are responsible for ensuring that your contact details (mobile phone number and email address) are accurate to receive appointment confirmations and intake forms.

3. Cancellation, Rescheduling & No-Show Policy

We reserve dedicated provider time and treatment rooms specifically for your scheduled procedure.

• Notice Requirement: We require at least 48 hours' notice for extensive laser, deep resurfacing, or combined procedure appointments) to cancel or reschedule without penalty.
• Late Cancellations & No-Shows: Cancellations made inside the required window, or failure to appear for a confirmed appointment, are subject to a cancellation fee of $175 (or forfeiture of the applicable booking deposit), charged to the card on file.
• Late Arrivals: If you arrive more than 15 minutes late, your treatment may need to be shortened or rescheduled to respect subsequent patients' reserved time, and late cancellation fees will be applied in the amount of $300.

4. Treatment Packages, Neurotoxin Banks & Expiration Policies

• Validity: Unless explicitly stated otherwise in promotional literature, all pre-paid treatment packages and Neurotoxin Banks (e.g., 50-Unit or 100-Unit Banks) are valid for twelve (12) months from the date of purchase.
• Provider-Specific Tiers: When Neurotoxin Banks or packages are purchased under provider-specific tiers (e.g., Ana & Cristina vs. Katerina Coming, NP), treatments must be redeemed with the designated provider or tier, unless an upgrade fee is settled prior to administration.
• Non-Transferability: Treatment packages, banking allocations, and special promotions are non-transferable between individuals and cannot be redeemed for cash or credit toward unrelated services.
• No Refunds: All delivered medical and aesthetic services, injected products (neurotoxins, dermal fillers, biostimulators), laser pulses, and partially utilized treatment series are non-refundable.

5. Patient Intake, Medical History & Informed Consent

Prior to receiving any clinical treatment, you must complete all required health history intake documentation and clinical informed consent forms. You agree to provide complete, truthful, and accurate information regarding your medical history, current medications, allergies, previous aesthetic interventions, and pregnancy or nursing status. REJUV Medical reserves the right to decline or postpone treatment if a clinical contraindication is identified.

6. SMS Communications & A2P 10DLC Terms

By providing your mobile phone number to REJUV Medical via our website, Vagaro booking, intake paperwork, or SMS opt-in, you consent to receive transactional and promotional text messages.

• Message Types: Appointment reminders, intake documentation links, post-care instructions, clinic updates, and marketing specials.
• Message Frequency: Message frequency varies based on your appointment cadence and promotional preferences.
• Rates: Standard message and data rates may apply depending on your wireless carrier plan.

• Privacy: We respect your privacy. Mobile numbers and SMS consent records are not shared, sold, or rented to third parties for affiliate or external marketing purposes.

7. Intellectual Property

All content, logos, visual graphics, clinic photos, text, and software present on https://rejuvsf.com are the exclusive property of REJUV Medical or its licensors and are protected under United States and international copyright, trademark, and intellectual property laws.
